Treatment Options for Surgery for Kidney Stone in Singapore

A kidney stone attack can strike without warning, bringing intense pain in the side or back, nausea and blood in the urine.

While many small stones pass on their own, larger or stubborn stones may need medical procedures.

If you have been told you may need surgery for kidney stone in Singapore, it helps to understand the options, how doctors choose between them and what to expect for recovery and costs.

This guide explains the main treatments available in Singapore hospitals and clinics, from watchful waiting to minimally invasive procedures.

It also covers cost benchmarks, MediSave and insurance considerations and practical tips for preparing for your consultation.

When Is Surgery Needed?

Many stones smaller than about 5 mm pass naturally with fluids and pain relief. A doctor will usually recommend treatment when:

• The stone is too large or in a position where it is unlikely to pass
• It is blocking urine flow or causing swelling of the kidney
• There is an infection together with a blocked kidney
• Pain, vomiting or bleeding cannot be controlled
• You have only one kidney or reduced kidney function
• The stone keeps growing or causes repeated symptoms
• Your work or travel makes it unsafe to wait, for example for pilots or frequent flyers

Severe pain with fever, chills or an inability to pass urine is an emergency. Call 995 or go to the nearest emergency department without delay.

First Steps: Tests and Observation

Before choosing a treatment, your urologist will usually arrange imaging, most often a non-contrast CT scan or ultrasound, together with urine and blood tests.

These show the size, number, and location of the stones, how hard they likely are, and whether the kidney is affected.

For small, uncomplicated stones, your doctor may suggest observation with fluids, pain relief, and sometimes medication that relaxes the ureter to help the stone pass.

This approach is not suitable if there is infection, severe obstruction or poorly controlled pain.

Extracorporeal Shock Wave Lithotripsy (ESWL)

ESWL uses focused shock waves, generated outside the body, to break the stone into small fragments that pass in the urine. It does not require any cuts.

Best for: Small to medium stones, generally up to around 2 cm, in the kidney or upper ureter.

What to expect: It is usually done as a day procedure with pain relief or light sedation. Blood in the urine and mild discomfort as fragments pass are common. Some patients need more than one session.

Limitations: It may be less effective for very hard stones, stones in certain parts of the kidney and in people with a higher body weight. Blood thinners may need to be adjusted beforehand, so tell your doctor about all the medication you take.

Ureteroscopy and Laser Lithotripsy

In ureteroscopy, a thin scope is passed through the urethra and bladder into the ureter, and sometimes into the kidney.

The surgeon locates the stone and either removes it with a tiny basket or breaks it with a laser.

When the scope reaches inside the kidney, the technique is often called retrograde intrarenal surgery (RIRS).

Best for: Ureteric stones and many small to medium kidney stones, including those that did not respond to ESWL.

What to expect: It is performed under anaesthesia, and there are no skin incisions. A temporary stent may be left in the ureter for a short time to help healing and drainage. Stents can cause urgency, frequent urination and mild discomfort, and they must be removed at the scheduled time.

Percutaneous Nephrolithotomy (PCNL)

PCNL is reserved for large or complex stones. The surgeon makes a small opening in the back, creates a narrow channel into the kidney and uses a nephroscope to break up and remove the stone directly.

A small tube may remain in the kidney for a short time afterwards.

Best for: Stones larger than about 2 cm, branching staghorn stones and stones that did not clear with other methods.

What to expect: It requires general anaesthesia and a hospital stay. It is very effective at clearing a large stone burden, but because it enters the kidney directly, it carries a higher risk of bleeding and infection than the other options.

Open or Laparoscopic Surgery

Traditional open surgery is now rarely needed. It may be considered when other methods are unsuitable, for example with unusual anatomy, extremely large stones or the need to correct a related structural problem.

Laparoscopic and robotic approaches use small incisions but are also reserved for special situations.

How Do Doctors Choose the Right Procedure?

There is no single best treatment. Your urologist will weigh:

• Stone size, number, position and hardness
• Whether the stone is causing blockage or infection
• Your kidney function and overall health
• Anatomy of your urinary tract
• Whether you take blood thinners or have other medical conditions
• Previous treatments and your preferences

In general, smaller stones favour ESWL or ureteroscopy, while large or complex stones often need PCNL.

Costs and Financing in Singapore

Cost is a key concern for many patients. The Ministry of Health publishes bill benchmarks for common procedure codes, including ureteroscopy, retrograde intrarenal surgery, PCNL and ESWL.

These show typical and range bills drawn from actual transacted bills for Singapore Citizens, inclusive of GST but before MediSave and insurance payouts. Public hospital amounts are after Government subsidies.

For example, MOH data for ureteroscopy at a public hospital as a subsidised day surgery show typical bills around S$1,600, though ranges are wide and vary by hospital, ward type, stent insertion, and complexity.

Private hospital bills are usually considerably higher. For anyone planning surgery for kidney stone in Singapore, the best approach is to ask the clinic for a written, itemised estimate that includes the surgeon's fee, anaesthesia, hospital charges, stent insertion and follow-up care.

Coverage options may include:

• MediSave: Kidney stone procedures are generally claimable, subject to the withdrawal limit for each procedure category.
• MediShield Life: Provides basic coverage for large hospital bills, subject to its claim limits and deductibles.
• Integrated Shield Plans: May extend coverage to private hospitals and higher ward classes, depending on the policy.

Rules and limits change, so confirm the current figures with your insurer, the hospital's billing office or the CPF Board before treatment.

Preparing for Your Consultation

• Bring your scans, reports and a list of your medications
• Note how long you have had symptoms and what makes them better or worse
• Mention any previous stones, family history and conditions such as gout or diabetes
• Ask about all treatment options, including non-surgical choices
• Ask about expected hospital stay, stent use and time off work
• Request an itemised cost estimate

Recovery and Aftercare

Recovery depends on the procedure. After ESWL or ureteroscopy, many people return to normal activities within a few days to a week, though you may notice blood in the urine and mild discomfort. After PCNL, recovery is longer and you may need to avoid heavy lifting for several weeks.

Drink plenty of fluids unless told otherwise, take pain relief as prescribed and attend all follow-up appointments, especially if a stent needs to be removed. Contact your doctor if you develop fever, heavy bleeding, severe pain or difficulty passing urine.

Preventing Kidney Stones From Returning

Stones often recur, so prevention matters. Staying well hydrated is the single most important step, particularly in Singapore's warm climate. Your doctor may also advise limiting salt, moderating animal protein and adjusting foods high in oxalate depending on your stone type. Testing the stone and your urine can identify specific risk factors and guide medication if needed.
